Frequently Asked Questions - SDR Corrective Actions

The following are Supply Discrepancy Reports (SDR) Responsibilities for Corrective Actions. This information should help you determine what needs to be done and within a specific timeframe.

Unit level receiving, upon receipt of damaged or under packaged materiel, should:

  1. Analyze and determine the discrepancy.
  2. Fill out SF 364 and submit through proper channels.
  3. Initiate follow-up SDR if no notice of action is received within 45 days.

Action activities, upon receipt of SF 364, should:

  1. Immediately investigate.
  2. Provide statement of corrective action to the control point and the reporting activity within 25 days.
